Describe steps taken to ensure that the PPG is representative of the practice population in terms of gender, age and ethnic background and other members of the practice population:

 

The practice has taken numerous steps to increase membership of our PPG including; advertising on the Practice website, Practice newsletter, notice board, posters, adding slips to prescriptions and personal invitation.

Our demographics show a wide age range and our practice participation is similar in makeup. We will continue to try to attract further members to the group.  

 

 

Are there any specific characteristics of your practice population which means that other groups should be included in the PPG?
E.g. a large student population, significant number of jobseekers, large numbers of nursing homes, or a LGBT community?

  1. Action plan priority areas and implementation

 

Priority area 1

 

Description of priority area:

 

Capacity & Demand; many positive messages received around capacity in comparison to local practices. Triage was again suggested as a method to maintain good capacity and demand management.

 

 

What actions were taken to address the priority?

 

Triage has been run for a trial period once before and the findings were mostly positive. Further trial will take place once staff roles are full (see priority area 3)

 

 

 

 

 

Result of actions and impact on patients and carers (including how publicised):

 

Triage will reduce waiting times for appointments; aid patient education as unnecessary appointments can be addressed at point of request, ensure correct clinician is allocated to the appointment.

Increasing staff development will have a positive effect on capacity (see priority 3)

Priority area 2

 

Description of priority area:

Dementia; To follow the local campaign to raise awareness of dementia.

 

 

 

 

What actions were taken to address the priority?

Audits were undertaken to help identify patients with possible dementia

All staff were trained as ‘Dementia Friends’

Dementia Friends ‘Master classes’ were advertised to patients to increase understanding within the community

 

 

 

 

 

Result of actions and impact on patients and carers (including how publicised):

Increased awareness promotes understanding and identification of Dementia in the community. Early intervention and referral can help treatment and care outcomes.

Publicised by notice in waiting room re classes, Joining of the Dementia Action Alliance network and face to face contacts.

 

 

 

 


 

Priority area 3

 

Description of priority area:

Increase / restructure of Practice team; To facilitate a better working pathway for capacity management development of the team to include new roles was necessary.

 

 

 

 

What actions were taken to address the priority?

We have developed our HCA, 2 practices nurses, widened the reception team and recruited a Salaried Gp to the practice. This development of the practice team has allowed extra capacity and a diversification in workload to ensure appropriate patients are seen by the most appropriate member of the team.

 

 

 

 

Result of actions and impact on patients and carers (including how publicised):

 

Additional demand has been managed across the wider team and the increased level of access has received positive comments.

How has the practice made efforts to engage with seldom heard groups in the practice population? The group has been advertised in many different ways as described above to engage a wide range of patients.

How has the service offered to patients and carers improved as a result of the implementation of the action plan? Wider range of appointments available; appointment demand monitored and alternative methods of contact available; Increased awareness of Dementia in local area.
